Facility Evaluation Report

Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Charity Bailey, Administrator.

Today's visit was in response to an LIC 624 Incident Report, which licensee self submitted to the CCLD San Diego Regional Office (received on 04/16/2025), involving Client #1 (C1).

During today’s visit, LPA performed a brief facility tour, collected copies of pertinent records, and interviewed relevant staff. According to the LIC 624 C1 went to the hospital complaining of pain. They called 911 and went to the hospital on 4/15/2025. As of today's visit C1 is still in the hospital with no follow up reason as to why they are still there. Interviews revealed that C1 called the administrator and explained that they are in the behavioral unit at the hospital. Interviews revealed that C1 calls the facility daily. Administrator states the hospital has not called with any information.

No deficiencies were observed or cited during todays visit.

An exit interview was conducted with Charity Bailey, to whom a copy of this report, the Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during at the conclusion of the visit.
