Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Jose Gonzalez, Administrator.
Today's visit was in response to a LIC624 Incident Report which licensee self-submitted to the CCLD San Diego Regional Office (RO), regarding Client #1 (C1) being AWOL (absent without leave) from the facility. According to the LIC624 (received at the RO on 04-22-2026): staff observed C1 had AWOL'd shortly after being prompted to complete laundry. Staff followed C1 to a liquor store, where they were observed attempting to obtain cigarettes from the ground or from individuals outside the store. C1 entered the store quickly. Staff briefly lost visual contact for approximately three minutes but were able to locate C1 again inside the store. C1 exited shortly after and sat at a curb for approximately 45 minutes. Approximately around 7:50 PM, C1 entered a smoke shop (Gigs and Gars) and was able to obtain a lighter. The administrator arrived and picked up both C1 and staff at approximately 8:00 PM. At that time, C1 was compliant. Upon return to the residence, C1 requested to contact law enforcement and emergency medical services, stating they had injured them self by punching walls. Staff contacted Alvarado Psychiatric Institute to inquire about an evaluation; however, they reported no availability. While requesting emergency services, C1 stated that earlier in the day they had obtained and used marijuana "roaches" while at the beach during program hours and reported that they felt "very high" and needed medical attention.
