The inspector’s account

Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Jose Gonzalez, Administrator.

Today's visit was in response to a LIC 624 Incident Report which licensee self-submitted to the CCLD San Diego Regional Office (RO), regarding Client #1 (C1) being AWOL (absent without leave) from the facility on August 2, 2026, August 3, 2026 and August 4, 2026. According to the LIC 624s the client left all three times and returned. Staff searched the surrounding area and or called 911, C1 requested to be taken to the hospital. After one of the elopements the staff took C1 to the hospital after scraping their hand on a brick. There are no incidents surrounding the AWOLs although when then client gets upset or does not get their way that is when the clients wanted to AWOL.

C1 had a follow- up appointment with their psychiatrist on 08/06/2026 and they are in the process of changing the client's medications. Interviews with C1 revealed that they don't have a reason why they leave. Administrator states that they are working on getting the supplemental staffing reinstated due to the frequency of C1 AWOLing.

According to C1’s latest LIC 602 Physician’s Report dated 2/23/26, it states that the client is not able to leave the facility. C1 otherwise is independent with Activities of Daily Living (ADLs), able to follow instructions, able to communicate, and not confused. Licensee has a up to date absentee notification plan on file. No deficiencies cited during todays visit

An exit interview was conducted with Jose Gonzalez, Administrator, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during today’s visit.
