Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Juan Cesati, Staff and Jose Gonzalez, Administrator arrived during the visit.
Today's visit was in response to a LIC 624 Incident Report which licensee self-submitted to the CCLD San Diego Regional Office (RO), regarding Client #1 (C1) being AWOL (absent without leave) from the facility on June 11, 2026. According to the LIC 624 client was upset with staff and before they left they called 911. The sheriff arrived around 910 pm and the client left 20 minutes after the sheriffs arrived. Staff searched the surrounding area. They located C1 and asked them to return to the facility. C1 obliged and got into the administrators car smelling like marijuana. The administrator observed the client stumble and due to concern of intoxication, they called 911. C1 was taken to the hospital emergency room on 6/11/26 and they kept them under monitoring for altered mental status. Interviews revealed that on 06/14/26 the hospital discharged C1. The administrator made a follow up visit with their primary doctor that was scheduled on 06/30/2026.
According to C1’s latest LIC 602 Physician’s Report dated 2/23/26, it states that the client is not able to leave the facility. C1 otherwise is independent with Activities of Daily Living (ADLs), able to follow instructions, able to communicate, and not confused. Licensee has a up to date absentee notification plan on file. No deficiencies cited during todays visit
An exit interview was conducted with Jose Gonzalez, Administrator, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during today’s visit.
