Facility Evaluation Report

Licensing Program Analyst (LPA) Tiffany Holmes conducted a case management visit regarding facility closure. LPA went to the facility to conduct the annual visit and was greeted by, identified herself to, the new owner of the home.

During today's visit, LPA briefly toured the downstairs of the facility/home and verified that there were no clients in care. According to the new owner of the home, they purchased the home in December of 2025 and moved in January of 2026. LPA observed two small children, different furniture than the facility and a dog.

An exit interview was conducted and a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) will be mailed to licensee at their other facility.
