Complaint Investigation Report
Interviews with outside sources revealed the complaint was in regards to C1 having to wait for services/placement from the San Diego Regional Center (SDRC) into a new facility. Interviews with outside sources revealed the staff at the facility did all they could do for C1. Interviews revealed they did not have any issues with the facility and the care that was being provided for C1.
The Department has investigated the above-mentioned allegation and based on interviews, LPA observations, and records review, it was determined that the complaint allegation is Unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Joshua Moala via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
