Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Josh Moala, Administrator.
Today's visit was in response to a Special Incident Report (SIR), which Licensee self-submitted to the CCLD San Diego Regional Office (received on 04/04/2025), involving Client #1 (C1). [See LIC 811 Confidential Names List for a description of person identifiers used in this report].
During today’s visit, LPA performed a facility tour and welfare check. (On 04/04/2025 LPA verified that C1 and their house mates were safe by phone, Administrator stated that both C1 and Staff 1 (S1) were taken away from the facility.) LPA collected copies of pertinent care records and conducted interviews. On 04/03/2025 Interviews revealed that S1 was transporting C1 downtown when C1 began playing music at an excessively high volume. Despite multiple requests from staff and other clients to lower the volume, C1 did not. Interviews revealed that due to the discomfort experienced by others, interviews revealed that S1 pulled over to address the situation. At that time, the Administrator was on the phone with C1, explaining that the loud music was causing distress to the other clients and suggested alternative coping strategies.
