Facility Evaluation Report

Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced Case Management visit to the facility. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Executive Director (ED) Scottie Geno and Director of Assisted Living (DOAL) Brittany Blaul. LPA had intended to conduct the facility's required Annual Inspection, but realized that the facility had already undergone one in January 2026.

LPA, ED Geno, and DOAL Blaul met for introductions and to discuss licensing resources. ED Geno and DOAL Blaul provided LPA a brief tour of the facility grounds, common spaces, and resident amenities.

No health and/or safety concerns were noted during the visit and no deficiencies were cited during the inspection. An exit interview was conducted with Executive Director Geno to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
