Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. Upon arrival, LPA was greeted by Licensee McIntyre, identified herself, was granted entry into the facility, and informed of the purpose of the visit.
According to the facility’s license, the maximum capacity is six (6) residents. All residents may be non-ambulatory, one (1) may be bedridden, and the facility has a waiver to admit up to three (3) residents who receive Hospice care.
During today’s inspection, there were five (5) residents in care. One (1) resident is receiving hospice care and is bedridden. Of the remaining four (4) residents, three (3) are non-ambulatory.
LPA Correia, accompanied by Licensee McIntyre, toured the facility and inspected each room. The facility was observed to be clean, sanitary, and in good repair. Pathways were free of obstructions and slip hazards. Resident bedrooms contained the required furnishings. Extra linens, hygiene supplies, and Personal Protective Equipment (PPE) were available. Medications, toxins, and sharps were all locked and inaccessible to residents in care.
