Facility Evaluation Report
Licensing Program Analyst (LPA) Jose De La Cruz made an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit with S1, and later with Facility Administrator Eliza Pangan.
LPA arrived at 8:05 am, facility looks clean and in good repair and decorated for the Holidays. At 8:12 am LPA requested clients files and at 8:16 interacted with C1. At 8:24 am, LPA reviewed the clients files and at 8:37 am had another interaction with C1. At 9:50 am, LPA reviewed staff records. Licensee arrived at 10:10 am and LPA started the facility tour at 10:50 am. At 11:00 am, LPA and Licensee entered the garage, which was locked. Washing machine and dryer were working and chemicals and sharps were safely locked.
LPA and Licensee toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities.
