Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit for a Plan of Correction clearance. LPA Lopez identified herself and was allowed entry by Fey Estrella Manalo, caregiver. LPA met with Licensee Novelita Domdom and discussed the purpose of the visit.

On 01/09/2024, the facility was issued two deficiencies regarding the temperature of the hot water in that 5 out of 5 water faucets measured over the allotted 120 degrees Fahrenheit (F). The facility was also cited for the ambient internal facility temperature which was lower than the required temperature.

During today’s visit, LPA measured the hot water temperatures in all five faucets, and they measured as follows: bathroom #1 measured hot water at 113.2 degrees F; bathroom #2 measured hot water at 113.2 degrees F; bathroom #3 measured hot water at 113.5 degrees F; bathroom #4 measured hot water at 113 degrees F; kitchen sink measured hot water at 108.7 degrees F. The ambient room temperature measured between 69 - 70 degrees F. As such, the deficiencies have been corrected and are deemed to be cleared during this visit.

No deficiencies were cited during today's visit. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Licensee Novelita Domdom at the conclusion of the visit. The signature below confirms receipt of these documents.
