Licensing Program Analyst (LPA) Jesse Gardner and Licensing Program Manager (LPM) Joel Esquivel met with Administrator/Licensee Lolita Gatmaitan to provide complaint findings.
LPM Esquivel also explained that the facility is currently licensed and no probationary limitations are currently noted.
An exit interview was conducted where a copy of this report was discussed with and provided to Administrator Gatmaitan.
