Facility Evaluation Report
On 4/8/2025, Licensing Program Analyst (LPA), Janette Romero arrived unannounced to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Eddie Merete who was informed of the purpose of the visit. The facility has a fire clearance to serve six (6) ambulatory clients only. During the visit, LPA observed one (1) client and one (1) staff present and was informed four (4) clients were attending day program. Administrator, Leena Dost arrived during the visit.
LPA toured the facility with Caregiver Merete and observed the facility is made up a two-story home with four (4) client bedrooms, three (3) client bathrooms, a kitchen, dining room, living room, laundry room, and attached garage. Indoor and outdoor passageways are free of obstruction. Outdoor shaded seating is available for clients in care and no bodies of water were observed on the premises. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and seven-day supply of non-perishable foods. Caregiver Merete tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed a charged fire extinguisher in the kitchen that was last serviced on 5/16/2024. Medications are secured in a locked storage closet near the dining room. LPA reviewed the Medication Administration Record for two (2) clients while Caregiver Merete reviewed their physical medications and no discrepancies were discovered. Disinfectants and cleaning solutions are secured in a locked cabinet in the laundry room. LPA toured the garage and observed an emergency supply of food and water. LPA reviewed the facility's Fire Drill Log which noted their last fire drill was conducted on 12/26/2024. LPA reviewed the Record of Client's/Resident's Safeguarded Cash Resources (LIC 405) for all five (5) clients while Caregiver Merete reviewed their physical monies and no discrepancies were discovered. Client files reviewed had updated Individual Program Plans, physician's reports, and signed admission agreements. Staff present has a criminal record clearance, valid first aid/CPR certification, and is associated with the facility. The facility's certificate of liability insurance expires on 7/18/2025. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Dost.
