The inspector’s account

On 09/03/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. Licensee, Leena Dost and Caregiver, Machona Merete arrived to the facility to meet with LPA. However licensee reported they had an important meeting and left the premises during the visit. The facility has a fire clearance to serve four (4) ambulatory adults and LPA was informed all four (4) clients were out in the community attending day program during the visit.

LPA toured the facility with Caregiver Merete and observed the facility is made up of a one-story home with two (2) client bedrooms, two (2) restrooms, a kitchen, dining area, living room, and attached garage. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had nonskid mats in the shower. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked hallway closet. Caregiver Merete tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen last serviced on 05/05/2025. Merete has a criminal record clearance and is associated with the facility. Client files revi ewed had updated Individual Program Plans and signed admission agreements. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources ( LIC 405) for all four (4) clients while Caregiver Merete counted their physical monies and no discrepancies were discovered. LPA reviewed the facility's Fire Drill Log noting their last fire drill was conducted on 07/30/2025. Clients' personal rights, complaint procedures, emergency phone numbers, and facility sketch are visibly posted near the dining room and front entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Caregiver Merete.
