Facility Evaluation Report
On 01/27/2026, Licensing Program Analyst (LPA) Janette Romero arrived unannounced to conduct a required annual inspection. LPA was greeted and granted entry by Direct Support Professional (DSP) Angel Diaz who was informed of the purpose of the visit. Administrator Yesenia Beltran was contacted over the phone and also informed of the purpose of LPA's visit. The facility has a fire clearance to serve four (4) ambulatory clients ages 18 through 59. During the visit, LPA did not observe any clients present in the facility and was informed they were out in the community attending day program and school. DSP Diaz has a criminal record clearance and is associated with the facility.
LPA toured the facility with DSP Diaz and observed the facility is made up of a one-story home with two (2) client bedrooms, a client bathroom, kitchen, dining room, and attached garage. No bodies of water were observed on the premises. Indoor and outdoor passageways were free of obstruction. Client bedrooms had the required bedding, furniture, and lighting. The client bathroom had a working toilet, toilet paper, paper towels and soap readily available. LPA also observed a non-skid mat in the shower. LPA observed additional hygiene supplies and bedding available for the clients in care. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are secured in a locked cabinet near the kitchen. Knives are secured in a locked kitchen drawer. LPA toured the garage and observed an emergency supply of food and water along with Personal Protective Equipment including gloves and gowns available in the facility. DSP Diaz tested one of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed two (2) charged fire extinguishers mounted in the facility that were last serviced on 09/02/2025. LPA reviewed the facility's Disaster Drill Log noting the facility's last fire drill was conducted on 01/15/2026.
