Facility Evaluation Report

On May 28, 2026, Licensing Program Analyst (LPA) Aziz Faizi arrived at the facility unannounced to conduct the required annual inspectio n and met with Program Director Flor Angel. The facility is licensed for 150 adults currently serving 95 clients.

LPA conducted a tour of the interior and exterior of the facility. The facility is a single level suite consisting of a music room, art room, (4) personal care rooms, nurse's office, kitchen and (4) activity rooms. There are 4 bathrooms to be used by staff and clients. The kitchen includes a kitchen for clients use. Clients are responsible for bringing their own meals, but complimentary snacks and beverages are provided during care.

The smoke and carbon monoxide detectors were tested by a local contractor monthly and all parts connected to the detectors were operable and found to be in good repair.

The hot water temperature was 105.4°F. Fire extinguishers equipped throughout the building are maintained in accordance with the department's safety requirements expiring on 12/01/2026. Adequate staff were present to supervise clients during the visit.
