Facility Evaluation Report
On 2/3/2026, Licensing Program Analysts (LPAs) Ahliah Sharp, Armando Perez and Aziz Faiza arrived announced to conduct a required annual required inspection. LPAs met with Executive Director Kecia Stinnett who assisted with todays visit. Inspections was announced due to the existing knowledge of prior fire.
On 9/24/2024, a fire erupted in the facility resulting in the home becoming uninhabitable. As noted in prior annual conducted on 2/24/2025, ED reported the clients who resided in the home during the fire incident were immediately and appropriately relocated, and the home is not currently occupied. LPAs and ED conducted a tour of the inside of the first and second floor of the home and garage.ED reported the cause of the fire was due to a gas leak. Perishable and non-perishable food was discarded after the fire incident and will be replenished when the home becomes habitable. Client and Personnel files could not be reviewed. Smoke and carbon monoxide alarms could not be inspected and observed to be missing in areas of the facility.
During the tour, LPAs observed there is no electricity or gas available in the facility and ED reported this was a result of the fire damage. ED added the facility requires several permits and repairs before it is safe for clients to return to the home. Before any clients are readmitted to the home, ED was reminded and is aware that another inspection will need to occur and they will need to contact the Riverside Regional Office for an additional inspection to ensure the facility meets licensing standards. At this time, ED does not have an estimated date of when clients will be able to return to the facility. ED added any updates they receive will be provided to the Department.
An exit interview was conducted and a copy of this report was reviewed and provided to facility representative.
