Facility Evaluation Report

On 8/14/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to address a deficiency discovered during the visit. LPA met with Executive Director, Kecia Stinnett who was informed of the purpose of the visit. During a complaint investigation, LPA requested Client 1's (C1's) record of cash resources. However, ED Stinnett reported C1's record of cash resources was unavailable for review at the time of the visit because it has been removed from the facility and placed in their administrative office which is off-site. As a result, the facility was cited. An exit interview was conducted and a copy of this report, LIC 809-D, Confidential Names list (LIC 811) and Appeal Rights were reviewed and provided to ED Stinnett.
