The inspector’s account

On 4/15/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator, Cynthia Herendeen who was informed of the purpose of the visit. The facility has a fire clearance to serve 200 ambulatory adults, of which 20 may be non-ambulatory. During the visit, Administrator Herendeen reported there was 116 clients and 48 staff present. LPA was informed the facility is a licensed vendor of the San Diego Regional Center.

LPA toured the facility with Administrator Herendeen and observed the facility is made up of two buildings with several program spaces, administrative offices, and two (2) kitchens. The program spaces include a theater, computer room, music and dance studios, several arts and crafts studios, and access to an equestrian center. LPA toured the kitchens and observed both areas to be clean. LPA was informed clients provide their own snacks and lunches to consume during day program hours. Administrator Herendeen reported the facility provides snacks/food when celebrating holidays and special occasions. Restrooms were clean and had toilet paper, soap, and paper towels readily available for client use. Cleaning solutions and disinfectants are secured in locked storage closets inaccessible to clients. Client medications are secured in a locked cabinet stored inside the nurse's office. LPA reviewed random digital client files. Client files reviewed had an updated physician's reports, Individual Program Plans, and signed admission agreements. The smoke alarms/carbon monoxide detectors were tested using a smoke aerosol can and LPA heard them to be operational. LPA also observed the fire alarm strobes to be operational. LPA observed several fire extinguishers charged and mounted throughout the facility that were last serviced on 11/25/2024. LPA reviewed the facility's Evacuation Drill Report noting their last fire drill was conducted on 11/14/2024. Facility sketches are posted near exit doors. During today’s visit, LPA did not observe any issues or concerns. An exit interview was conducted, and this report was reviewed and provided to Administrator Herendeen.
