Facility Evaluation Report

On 10/27/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced case management - incident visit for the purpose of following up on an unusual incident report submitted to Community Care Licensing by the facility. LPA met with Executive Director, Amber Rodgers, and explained to Amber the purpose of the visit. On 10/21/2025, the facility reported that Resident #1 (R1) was alleging an unknown male entered into R1's bedroom at night and did inappropriate acts to R1.

During the visit, LPA conducted an interview Executive Director Amber and R1. R1 granted LPA entry into their unit. LPA conducted observations and took pictures of R1's unit. In addition, LPA obtained copies of pertinent documentation. During a tour of R1's bedroom, LPA observed various hazards that do not follow code enforcement. LPA will be crossreporting concerns to appropriate agencies..

An exit interview was conducted and a copy of this report was provided to Executive Director, Amber Rodgers.
