The inspector’s account

On 11/8/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility for a required annual inspection. LPA was greeted and granted entry by Staff, Maribel Nambo who was informed of the purpose of the visit. Licensees, Lorena Maharaj and Sarai Marcelin arrived during the visit.The facility is a licensed vendor of the San Diego Regional Center. During the visit, there was 32 clients and 13 staff present.

LPA toured the facility’s interior and exterior with Staff Nambo. The facility is made up of three (3) classrooms, eight (8) bathrooms, two (2) staff offices, a changing room, sick room, conference room and several storage closets. Snacks are provided to clients during program hours. An emergency supply of snacks are stored in one (1) of the staff offices. Classrooms support socialization, independent and adaptive skills. Restrooms were clean and had toilet paper, paper towels and soap readily available for clients. Cleaning solutions/supplies and chemicals were secured in a storage closets, inaccessible to clients. Client medications are secured in a file cabinet in one (1) of the locked staff offices. LPA reviewed the Medication Administration Record for two (2) clients while Staff Nambo reviewed their physical medications and no discrepancies were discovered. LPA also reviewed random staff and client files. Staff present have a criminal background clearance and are associated with the facility. Staff files reviewed had a valid first aid/cardiopulmonary and non-violent crisis intervention certification. Client files had an updated physician's report, Individual Program Plans and signed admission agreements. The facility’s last fire drill was conducted on 11/7/2024. Licensee Marcelina tested the smoke alarms/carbon monoxide detectors and LPA observed them to be operational. LPA also observed six (6) charged fire extinguishers mounted throughout the facility, last serviced on 10/3/2024.

During today’s visit, LPA did not observe any issues or concerns. An exit interview was conducted and this report was reviewed and provided to Licensees.
