Facility Evaluation Report

Licensing Program Manager (LPM) Sabel Martinez and Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced Case Management visit. They introduced themselves and disclosed the purpose of the visit to Executive Director Stephanie Boudreau.

During the visit, an amended report was delivered and report signatures were secured.

An exit interview was conducted with Executive Director Boudreau, to whom a copy of this report, and Licensee Rights (LIC 9058), were provided via email. An email read receipt confirms the documents were received.
