Facility Evaluation Report

On 12/9/25, an informal meeting was held at the San Diego Regional Office. In attendance were Licensee/Administrator Todd Brooks, Facility Director of Operations Marilyn Ancho, Licensing Program Analyst (LPA) Arian Golbakhsh, Licensing Program Manager (LPM) Robyn Clark, and Regional Manager Jerry Romero, who joined virtually.

During the meeting, the department addressed the licensees’ questions, provided consultation on the Health & Safety Code and the California Code of Regulations, Title 22, and discussed ongoing collaboration with the department.

In addition, LPM Clark explained Licensee rights, including the right to appeal, the right to jointly develop plans of correction, and, the availability of waivers where applicable. LPM introduced the department’s optional Technical Support Program and distributed related materials.

A copy of this report, laws and regulations discussed, and licensee appeal rights were provided to License/Administrator Brooks, whose signature below confirms receipt of these documents.
