Complaint Investigation Report
Records reviewed during the investigation reflected that, with the exception of one male, male clients who reside in the facility are able to leave the facility unsupervised and without assistance. Accordingly, staff are not responsible for supervising behavior that clients engage in or display while outside of the facility or in the community. The investigation did not yield evidence to conclude that the one male client who requires supervision when away from the facility engaged in the reported behavior.
Based upon the foregoing, the allegation is unsubstantiated. This finding means that although the allegation may have happened or may be valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Froilan Montes, Administrator, and copies of this report and Licensee/Appeal Rights (LIC 9058) were provided to the administrator at the conclusion of the visit. His signature below confirms receipt of the rights and a copy of this report.
