Finally, and interview with the San Diego Regional Center (SDRC) Coordinator assigned to this facility revealed that they conducted a site visit on 03/19/25 and witnessed C1 eating a snack during the visit. There is insufficient evidence to support this allegation.

It was also alleged that staff prevent C1 from using the bathroom when needed. A review of C1’s medical report dated June 17, 2024 revealed that C1 is able to advocate for self and can care for their own toileting needs. An interview with C1 revealed that C1 reported having no issues having access to the bathroom. An interview with Outside Source #1(OS1), who advocates for C1, revealed that C1 is totally independent with toileting and has never complained about not having access to the bathroom. Interviews with (2) other clients revealed that they had no problems having access to the bathroom and have never witnessed staff preventing any clients from using the bathroom. Interviews with three (3) staff revealed that there is no facility policy preventing clients from using the bathroom and they have never witnessed C1 being prevented from using the bathroom. There is insufficient evidence to support this allegation.

It was also alleged that staff leave C1 outside of the facility for extended periods of time and that staff threaten C1. An interview with the San Diego Regional Center (SDRC) Coordinator assigned to this facility revealed that there are no concerns with staff mistreating C1. The coordinator also stated that C1 advocates for themselves and has no problems reporting issues to SDRC. The Coordinator also stated that they believe the facility provides proper care and supervision for the clients. An interview with C1 revealed that C1 stated that they were treated with respect by facility staff and have never been locked out of the facility. An interview with OS1 also revealed that they speak daily to C1 and visit C1 at the facility often, and they believe facility staff treat C1 and the other clients with respect. Interviews with two (2) clients also revealed that they have never been threatened by staff nor locked out of the facility. There is insufficient evidence to support these allegations.

Finally, it was alleged that the Licensee does not ensure that staff are present at the facility at all times. Interviews with three (3) clients revealed that staff are present at the facility at all times. Interviews with (3) staff revealed that staff are trained and instructed to provide 24 hour care and supervision for the clients and have never witnessed staff not being present at the facility.
