The inspector’s account

Licensing Program Analyst (LPA) Amy Domingo conducted an unannounced case management visit. Upon arrival, LPA was greeted by staff member Sabrina Febre, to whom she identified herself and explained the purpose of the visit. During the visit, LPA observed residents in care, completed a health and safety check, reviewed facility records, and conducted interviews with staff and residents.

The purpose of today’s visit was to follow up on a self reported incident. On May 7, 2026, the Department received an incident report regarding an event that occurred on May 4, 2026, in which staff found Resident 1 (R1) using the stove and attempting to position their head toward the burner. R1 sustained a minor burn to the front portion of their hair; no skin burns were noted. Staff immediately redirected R1 away from the stove. The incident was reported to the San Diego Regional Center (SDRC) service coordinator, and an assessment of R1’s needs was requested. On May 6, 2026, the SDRC service coordinator assessed R1 in the home, approved a supplemental 1:1 support hour, and recommended that R1 be evaluated by their psychiatrist. Staff will continue to monitor R1’s behaviors and assess the appropriateness of the current placement.

Interviews with staff indicated that R1 was seen by their psychiatrist, who increased R1’s current medication and prescribed an additional medication expected to arrive today. SDRC will also be providing behavior training to staff this weekend regarding resident behavioral needs. The facility reported that safety protection knobs will be installed on the stove. Staff will continue to provide redirection and closely monitor R1.

No deficiencies were cited during today’s visit. An exit interview was conducted with caregiver Sabrina Febre, who was provided with a copy of this report and the Licensee Appeal Rights (LIC 9058 03/22).
