Licensing Program Analysts (LPA) Dang Nguyen and Alyssa Ramirez conducted an unannounced Case Management visit. LPAs were welcomed by, identified themselves to, and discussed the purpose of the visit with Licensee Soky Nou.
LPAs briefly toured the facility, performed a health and safety welfare check on clients in care, and spoke with staff. No deficiencies were observed or cited on this date.
An exit interview was conducted with Nou, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 01/16) were provided.
