Complaint Investigation Report
The administrator confirmed R1 has a Major Neurocognitive Disorder and explained R1 would constantly interrupt staff while they are busy providing care. Therefore, staff may end up raising their voice out of frustration. However, the staff are good caregivers and treat the residents with dignity. The administrator denied witnessing S1 yell at R1 to go to bed.
Based on interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California code of Regulations, Title 22, Division 6 & Chapter 8, is being cited on the attached LIC 9099D. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 01/16) were provided to Staff, Petra Galindez whose signature below confirms receipt of these rights.
