Facility Evaluation Report
Licensing Program Analyst (LPA) Iby Strong conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. According to the facility’s license, the facility has a maximum capacity of six (6) clients, all of whom must be ambulatory. LPA arrived to the facility and no clients or staff were present. Administrator Kristen Hollobaugh arrived shortly after. Facility is actively vendorized by San Diego Regional Center.
On or around April, 2025, the Licensee informed the Department that the facility would be under construction after a kitchen and bathroom leak were identified. LPA Strong confirmed all clients have been temporarily placed in other DSC homes with an appropriate level of care.
As of today no clients or staff were present. LPA Strong toured the facility and observed construction throughout the inside of the home. No records were reviewed. No deficiencies were issued on today's date.
An exit interview was conducted with Administrator Kristin Hollobaugh, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
