Facility Evaluation Report
Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced required Annual Inspection. The facility file was reviewed prior to the visit. LPA Lopez identified herself, was granted entry by caregiver Zahara Musse. LPA discussed the purpose of the visit with licensee Safiyo Jama who later arrived and joined the visit. Caregiver Musse left during the inspection.
According to the facility’s license, there may be a maximum of four (4) clients, all of whom may be ambulatory, at any given time at the facility site. During today’s inspection, the facility’s current census is 2 clients living at the facility. There was one (1) client present at the facility site during the inspection and the second client who later arrived from their respective day program during the visit.
LPA, accompanied by licensee Jama, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. Extra linens and hygiene supplies were present. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and activities. Facility fireplace was inaccessible to clients.
The facility’s ambient internal temperature was comfortable and compliant, at 73 degrees (º) Fahrenheit (F). Hot water temperatures at taps accessible to clients became compliant during the visit: kitchen sink initially measured hot water at 135.5ºF and later measured at 113.4; sink in restroom #1 delivered hot water at 113.7 but ended at 105.1ºF; sink in restroom #2 delivered hot water at 128.8ºF which later measured at 105.1ºF.
