Facility Evaluation Report

Licensing Program Analyst (LPA) Liliana Silveira conducted an unannounced visit to provide follow-up consultation for a Plan of Correction (POC) visit. LPA explained the purpose of the visit and was allowed entrance into the facility by Care Staff Antoinette Saldan. Administrator Ana Saldan arrived shortly after.

LPA discussed the most recent citations issued during an Annual Inspection on February 5, 2026 with Ana and Antoinette. LPA suggested participation in the Community Care Licensing (CCLD) Technical Support Program (TSP) and Ana has agreed to participate.

No citations were issued during today's visit. An exit interview was conducted with Ana, to whom this report and LIC 9058 (03/22) were provided. Signature below confirms receipt of these documents.
