Facility Evaluation Report

Licensing Program Analyst (LPA) Nacole Patterson conducted an unannounced Case Management visit to the facility. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Lead RBT Cassandra Eversole. Today's visit was in response to the facility's pending closure, and to confirm relocation progress for the clients in care.

During today's visit, LPA conducted a wellness check at the facility and no health or safety issues were identified. LPA spoke with staff to obtain updates about the pending client relocations. One client was being moved out during the visit; LPA observed staff packing the client's items and the transport van waiting on site.

No deficiencies were cited during the inspection. An exit interview was conducted with Supervisor Direct Care Lead Trevor Sparkman, whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
