Complaint Investigation Report
There is one staff person who has the same name as the second name provided; however, through interview, R1 expressed that the person who allegedly engaged in the behavior does not work at the facility and expressed that the person works somewhere else. Other interviews conducted yielded no information to corroborate the allegation.
Due to a lack of sufficient evidence to conclude otherwise, the foregoing allegation is unsubstantiated. This finding means that although the allegation may have happened or may be valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Faizan Jamil, and a copy of this report was provided to the Business Director, whose signature below confirms receipt of copies of this report and Licensee Rights (LIC 9058).
