Facility Evaluation Report

On 04/14/25, at approximately 11:45PM, Licensing Program Analyst (LPA) David Roman conducted an unannounced Case Management Inspection. LPA D. Roman identified himself to Joanna Zamora, and discussed the purpose of the visit. At a later time, Administrator, Miriam Silva, was present along with Assistant Administrator, Holly Goolsby.

During today's visit LPA reviewed incident reports for C1 regarding multiple elopements. CCLD received incident reports which reported C1 eloped on 01/01/25, 01/03/25, 01/07/25, 01/08/25, 01/10/25, 01/11/25, 01/20/25, 02/10/25, 02/04/25, 02/18/25, 02/24/25, 03/02/25, 03/03/25, 03/04/25, 03/06/25, 03/07/25, 03/13/25, 03/19/25, 03/20/25, 03/21/25, 03/26/25, 03/29/25, 03/30/25, 03/31/25, and 04/01/25, 04/05/25, 04/09/25, 04/10/25, and 04/13/25. Injuries reported include suicidal ideation, aggressive bxs towards staff, aggressive bxs towards self, property damage, law enforcement responses, and CPI holds. LPA reviewed resident records and interviewed staff. No deficiencies were cited during today's visit.

An exit interview was conducted with Program Administrator, Miriam Silva, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit.
