Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA D. Roman met Operations Manager, Jennie Alshouse to which LPA identified himself to and discussed the purpose of today's visit.
LPA D. Roman along with facility staff toured the interior and exterior of the facility. LPA D. Roman randomly inspected 7 of the 13 rooms. The facility was sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. There were no toxic chemicals/poisons accessible to clients. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities.
In the kitchen area cooking/dining equipment and utensils were present. Refrigerators and freezers had sufficient food for about a week or more. Non-perishable food was checked for expiration dates. Medications were labeled, as required, and stored in locked areas. Water temperature was measured at 120 degrees F. One body of water, pool was located in the middle of the premises with a 5 foot fence surrounding it, gate was locked. Per Operations Manager no firearms or ammunition are kept at the facility. Fire Alarm and carbon monoxide detectors were in working condition. LPA observed emergency lighting and facility telephone in working conditions. Fire extinguisher(s) were present and inspected on 02/18/2025. First aid kit(s) were complete and readily accessible.
Resident records reviewed had required documentation. Staff records reviewed and also maintained required documentation. Some staff CPR certificates were up to date and the expired ones are going to be trained within the next month May 01, 2025, according to Operations Manager.
An exit interview was conducted with Operations Manager, Jennie Alshouse, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit. Their signature acknowledges the receipt of documents.
