Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to conduct the continuation of the required annual inspection. LPA was greeted by Administrator Sherwin Marcos, identified herself, and was granted entry into the facility. The current census was five (5) residents in care.
LPA Correia conducted the remainder of the facility tour during today's visit. The facility ambient temperature was 78 degrees Fahrenheit. Resident bathrooms and bedrooms were equipped with the required furnishings and safety measures. Medications, toxins, and sharp objects were kept in locked cabinets and inaccessible to residents in care. The facility has a shaded outdoor area and a large common area for resident activities. LPA observed the required amount of food appropriately stored and labeled. Per Administrator Marcos there is no ammunition or weapons kept on the premises. Carbon monoxide, smoke detectors, and fire extinguisher were present, operational, and or up to date.
LPA observed the required postings. Administrator Marcos’s certification is active. The facility’s Limited Liability Corporation (LLC) and liability insurance were active. The last disaster drill was conducted on July 21, 2025.
Based on the inspection the facility was in compliance and no deficiencies were cited. An exit interview was conducted with Administrator Marcos, and a copy of this report along with Licensee/Appeal Rights (LIC9058 FAS 01/16) will be provided at the conclusion of the visit and signature on this form acknowledges receipt of the rights and a copy of this report.
