Facility Evaluation Report

Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced Required Annual Inspection at the facility. Upon arrival, LPA received no response at the facility entrance and subsequently contacted Licensee Felicity Arcangeli via telephone.

Per Licensee Arcangeli, there are currently no residents residing at the facility, and the facility is not operating at this time. However, the Licensee stated she is maintaining an active license and plans to begin providing care in the future. The Licensee also mentioned that the facility is currently undergoing renovations.

An exit interview was conducted with Licensee Felicity Arcangeli via telephone. During the interview, LPA reviewed the findings of the visit. No deficiencies were cited during today’s inspection. A copy of the report will be provided to the Licensee via email, and confirmation of receipt will be documented.
