Facility Evaluation Report

Licensing Program Analyst (LPA) Esther Miller cited deficiencies discovered during an unrelated complaint visit. LPA was granted entry after identifying herself by Katherine Carver, House Manager. House Manager signed report but could not stay and make a Plan of Correction with LPA.

On March 29, 2023, House Manager informed LPA that a client was expected to move out by April 6, 2023 after being given a 30 day notice to vacate. Administrator Michelle Wojciechowsky admitted to not submitting a copy of the notice to the Department.

Pursuant to the California Code of Regulations, Title 22, Division 6, deficiency is being cited on the attached LIC809D and a plan of correction was developed. An exit interview was conducted with House Manager and a copy of this report, along with Licensee/Appeal Rights (LIC9058 03/22), were provided. After the exit interview, House Manager had to leave the facility due to a prior engagement. POC was discussed over the phone with House Manager and caregiver.
