Facility Evaluation Report
Licensing Program Analyst (LPA) Correia conducted an unannounced Required Annual Inspection. LPA was greeted by Executive Director (ED) Watkins, identified herself, and discussed the purpose of the visit. According to the facility’s license, the facility has a maximum capacity of six (6) clients, all of whom may be non-ambulatory, and is approved for delayed egress with a hospice waiver for six (6).
A review of staff, resident, and facility records was completed and found to be current and up to date. The facility’s last disaster drill was conducted in February 2026, and liability insurance is current.
LPA Correia, accompanied by ED Watkins, toured the interior and exterior of the facility and inspected each room. The facility was sanitary and in good repair. Pathways were free of obstructions and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, toilets, and showers were all in working order and equipped with appropriate safety measures. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities.
