Facility Evaluation Report

LPA Correia conducted an unannounced visit to the facility to complete the required annual inspection. LPA identified herself and met with Executive Director (ED) Watkins. The facility is licensed for a capacity of 6 residents, however had a census of 0 during today's visit.

Interviews with ED Watkins revealed that the facility is not operating at this time. However, the ED stated that the license remains active and the facility will begin operating once capacity is reached at the other buildings located on the same property or within close proximity, and additional beds are needed. LPA, accompanied by ED Watkins, conducted a facility tour, confirming there were no residents in care.

An exit interview was conducted with ED Watkins, and LPA reviewed the findings of the visit. No deficiencies were cited during today’s inspection. ED Watkins was advised a copy of the report will be provided at the conclusion of the visit.
