Facility Evaluation Report

Licensing Program Analyst (LPA) Correia conducted an unannounced case management visit to check on the health and safety of residents in care. LPA gained access to the facility, identified herself, and met with the Administrator to discuss the purpose of today's visit.

LPA's visit consisted of briefly speaking with Administrator Yasar regarding the resident’s health and safety, and conducted a staff interview, ensuring plans were in place for the protection of the residents in care, concerning assistance with administration of medication and the conditions of the physical plant and maintaining the health and safety of residents in care.

LPA did not observe any immediate health and/or safety violations at the time of visit. No deficiencies were issued during today's visit. An exit interview was conducted with Caregiver Bartolome who was informed a copy of this report and Licensee rights will be provided at the conclusion of the visit.
