Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced Case Management visit. LPA met with Executive Director (ED) Azemikhah and discussed the purpose of the visit.
During today's visit LPA obtained a signatures on amended Community Care Licensing (CCL) report.
An exit interview was conducted with ED Azemikhah to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 01/16) will be provide.
