Facility Evaluation Report
LPA Correia conducted the required one-year inspection. Upon arrival, LPA was greeted by Caregiver Janet Balingit, identified herself, and explained the purpose of the visit. The facility is licensed to serve six residents aged 60 and above, all of whom may be non-ambulatory and/or receiving hospice services, with one (1) resident permitted to be bedridden. Administrator Jeremy Bagaoison arrived and joined the visit.
During today’s inspection, all six residents were present. Three residents are currently receiving hospice services, and the remaining three receive Home Health services.
A review of resident records confirmed that required documents including the current Physician’s Report, Resident Appraisal, Needs & Services Plan, Identification and Emergency Information, and Admission Agreement were complete and up to date. Personnel files were also reviewed and contained Criminal Record Clearance, required training, Health/TB Screening Reports, First Aid and CPR certification, and qualifications, all of which were current and complete. The Administrator’s certification is valid until March 13, 2026. The facility was in compliance with disaster drill requirements and maintains a current lease agreement and liability insurance.
