Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit. LPA D. Roman identified himself to Laura Cooke, Program Director, and discussed the purpose of the visit.
LPA D. Roman along with facility staff toured the interior and exterior of the facility and inspected bedrooms and bathrooms. The facility was sanitary, and in good repair. Pathways were free of obstruction and slip hazards. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client therapy activities.
Cooking/dining equipment and utensils were present, clients cook their own meals. The facility had sufficient food for clients in care. There were no toxic chemicals/poisons accessible to clients, nurse station has double locked medication cabinets and controlled medications are triple locked. No pools or bodies of water on the premises. Smoke alarms and fire extinguishers present throughout the facility. During today's visit LPA D. Roman interviewed facility staff and reviewed the residents record.
An exit interview was conducted with Program Director, Laura Cooke, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit.
