Facility Evaluation Report

Licensing Program Analyst (LPA) Rebecca Ruiz conducted a plan of correction visit. LPA was greeted by, identified herself to, and explained the purpose of the visit to House Manager Ramil Macaspac.

The purpose of the visit was to verify if the deficiency issued on 2/21/2024 had been corrected. On 2/21/2024, the licensee was issued a deficiency regarding over-capacity with a correction due date of 2/22/2024.

During today’s visit, LPA Ruiz reviewed clients' physician's reports and verified that the licensee corrected the deficiency regarding over-capacity for non-ambulatory clients by the due date of 2/22/2024.
