Facility Evaluation Report
Licensing Program Analyst (LPA) Ramin Hashemi conducted a visit to inspect the facility to approve a change of floor plan, allowing bedroom #4 to operate as a resident's bedroom from its previous designation as a caregiver's bedroom. The facility is licensed for six (6) non-ambulatory elderly residents ages 60 and up, one (1) of whom may be bedridden with a direct exit out of the building. LPA identified themselves and stated the purpose of the visit to Nora Salenga, Caregiver.
Prior to the visit, the LPA reviewed the application for the change in floor plan. A representative of Oceanside Fire Prevention Division inspected and granted fire clearance for the new floor plan on 12/05/2025.
During today’s visit, LPA conducted a tour of the facility, and observed no immediate health or safety concerns. LPA found physical plant to be consistent with the submitted facility sketch/floor plan. LPA observed resident rooms and accommodations to be clean, safe and in good repair.
An exit interview was conducted with Nora Salenga, Caregiver, and a copy of this report along with the Licensee Rights (LIC9058 03/22) was provided.
