Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced Case Management visit for a self-reported incident. LPA Lopez identified herself and was granted entry by the Facility Manager Linda Diaz. LPA stated the purpose of the visit and reviewed the basic elements of today’s visit with the Operations Director, Charlott Miller. Chief Operating Officer, Kenneth Gladstone, later arrived and joined the visit.

Today’s visit was in response to a self-reported incident that was called into the San Diego Regional Office on 04/24/2025. The facility reported an incident that transpired between client #1 (C1) and staff #1 (S1) on the morning of 04/23/2025. LPA requested and obtained relevant facility records pertinent to this incident. LPA informed Operation Director Miller and Chief Operating Officer Gladstone, that, at this time, the case requires additional telephone calls or visits relating to this incident. No deficiencies were observed or cited during this visit.

An exit interview was conducted with Operations Director, Charlott Miller, and Chief Operating Officer, Kenneth Gladstone, to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 01/16) were provided at the conclusion of the visit. The signature below confirms the documents were received.
