Facility Evaluation Report
Licensing Program Analyst (LPA) Hannah Rodgers conducted an unannounced case management visit to conduct follow up regarding a self-reported incident. LPA was greeted by, identified herself to, and explained the purpose of the visit with Administrator Michelle Ertel.
On March 20, 2025, the Department received an incident report that described that on March 19, 2025 Client #1 (C1) received the wrong dosage of their medication. C1 received a full dosage of a medication when they were prescribed to take half.
During today’s visit, LPA observed clients in care, reviewed facility records, and interviewed staff.
No deficiencies were cited on today’s date. An exit interview was conducted with Administrator Michelle Ertel, whose signature below confirms receipt of a copy of this report and the Licensee Appeal Rights (LIC9058 3/22).
