Facility Evaluation Report

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced Case Management visit for a self-reported incident. LPA identified herself, stated the purpose of the visit, and reviewed the basic elements of today’s visit with Program Director Melissa Martinez.

Today’s visit was in response to a self-reported incident that was submitted to the San Diego Regional Office on 3/13/26. The facility reported an incident that transpired between Client #1 (C1) and Staff #1 (S1) on 3/7/26. [See LIC811 Confidential Name List for a description of select person identifiers used in this report.] LPA requested and obtained relevant facility records pertinent to this incident. LPA informed the Program Director, that at this time the case requires additional visits and follow up relating to this incident. No deficiencies were observed or cited during this visit.

An exit interview was conducted with Program Director Melissa Martinez to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 01/16) were provided at the conclusion of the visit. The signature below confirms the documents were received.
