Licensing Program Analyst (LPA) Marisela Garcia -Centeno conducted an unannounced Health and Safety visit due to a reported death by family member, Daniel Daly of the licensee Ernestina Torres on Friday, June 12, 2026 . The facility has a licensed capacity of 3 ambulatory clients. During today’s visit, the facility had a census of 3 ambulatory clients, all of which were not present at facility. Per staff, the Licensee/Administrator for the facility Ernestina Torres had a sudden death on June 6, 2026 at the facility.
During the visit, LPA discussed with the licensee's designee, Daniel Daly the procedures for continuing to operate the facility. In addition, LPA confirmed with the placement agency that the three (3) clients will be relocated to a new licensed facility, Galindo Haven, 374604949 , located in Oceanside. A set meeting with Daly was scheduled for Tuesday, June 16, 2026 at 1:00 p.m. to discuss the Emergency Approval to Operate the facility.
No health and safety concerns were observed during the visit.
No deficiencies were cited during the visit. An exit interview was conducted with Daniel Daly to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
